I have anal fissures and in spite of persistent use of steroid ointments, vaso-dilator paste, suppositories I cannot get rid of them. After seven months of trying my doctor suggests surgery - sphincterotomy. Can you tell me something more about sphincterotomy? Will it finally remove those irritating anal problems?

Lateral internal anal sphincterotomy is one of those simple and safe operations where basically nothing can go wrong. After a day spent in hospital you will be able to leave home. This surgery is done under general anaesthesia. The surgeon will make a very small cut near your anus and divide your sphincter muscle. This simple procedure should enable your fissures to heal properly - the fissures cannot be removed, only the part called sentinal pile (if there is any). It’s a tag of swollen skin at the lower end of the fissure that actually guards it. There are no complications following this surgery (occasional short bleeding is possible), and the healing of the fissures comes very soon. It is important to reduce the chances of constipation right after the surgery since it can be very painful. Drink a lot of water, eat fibre - rich food and use stool bulking agent if necessary. Keep the area clean and dry and practise saline baths several times a day.

what is the cost a sphincterotomy? I am considering surgery after 8 month with an anal fissure.

A typical Anal Sphincterotomy Cost is going to be about $16k, but would probably end up being more if there were any complications.

RoyalPain- have you tried any of the other methods yet? Surgery should really be a last resort.

I just had an anal sphincterotomy and it didn't cost $16k. There was a $1400 fee for the surgery room, $565 for the procedure and about $450 for the general anesthesia. It's been a week and still have some pain and possible infection but not sure yet. It was a very easy procedure, in/out within 4hrs. Apart from the minor discharge I still have it's was worth it.

I've been dealing with my anal fissure for almost 4 years now. For the most part there are good days and bad day but I've been living with it cause I couldn't afford not to. Lately, I've been getting very sick after using the bathroom. Very high fever and painful headaches are only some of the problem. Are these things the consequences of having let this fissure fester?

I had a hemorrhoidectomy several years ago,(nothing I would recommend to anyone), the doctor was too aggressive and I now have anal stenosis. My sphincter muscle is too tight and will need sphincterotomy surgery to correct it. I am concerned about the recovery time, I like working out and wonder how long I will be out of the gym. If anyone can offer any words of wisdom I would certainly appreciate it.

I had a lateral internal sphincterotomy performed today. The procedure was outpatient im home now. Just wondering if anyone can shed light on the recovery. Right now feeling okay. Was told after three days i should be feeling better. Just trying to get some insight as far as what to expect tomorrow.. Thanks

I am a continence physio and would like to advise you to seek advise from a trained continence physio.  There is a LOT we can do to help with tight overactive sphincters, defecation problems, and persistent fissures.  Call you local continence physio and make sure they are familiar with these conditions.

Hello, I am wandering if you can help me. I have an embarrassing problem and I don't know if it can be helped. I am a 27 year old male. I have been diagnosed with IBS (constipation type) many years ago but recently I have seen a specialist who has been able to determine from an anal manometry that I have high anal canal pressure. I believe that this is the source of much of my problem. I do get fissures sometimes and these can be hard to heal. However the big problem for me is a social one. When I go to the toilet I am very loud (passing gas). I can't go quietly. This has caused me several problems. It means that I can't go when I'm at work which is a nightmare. It also means that I can't go to the toilet when anyone else is nearby due to the embarrassment. The result is constipation, isolation, depression, anxiety, etc. It is very hard to deal with, and I can't even talk to my doctor because it is such an embarrassing thing. However, I have been thinking recently that my problem might be to do with particularly tight sphincter muscles. This is why I am sending this message to you. I am expecting that my specialist doctor might refer me to a physio like yourself. However it is much easier discussing this anonymously. Is there anything that you can recommend that I do, or anyone that you could recommend that I speak to? If so I would be so grateful as I am in need of help, and I'm worried that my bowel problems will persist and maybe get worse if I don't do something about this. Thanks. R.

ps. I have tried many dietary changes, so I am not especially looking for advice on this front.

A few suggestions

A soft stool is essential for healing

I recommend 

Drinking 8 glasses of water 64oz in total

Take a fiber supplement twice a day like metamucil or other, after you take it drink one 8oz glass of water

Your diet should include mostly non animal food, no milk, no cheese, no eggs, no meat, no chicken, no seafood

For breakfast, 10 prunes in half a glass of water put in blender, one cup of raspberries and one cup of melon 

For lunch, Mashed potatoes with roasted mushrooms, one cup of pears with skin, two squares of chocolate as desert, only pure chocolate

For dinner, 10 prunes again, soup home made with spinach

Do not push your bowel movements, listen to your body

Do not have sex or masturbate for 8 weeks

Do not excercise, at the most just stretch lightly

Do not carry any weight, at the most 5 pounds

Walk slowly and do not dance or make quick movements

Do not consume alcohol or any other type of drug

Buy a product called Hemaway to help relax before you go to thebathroom

Take baths in lukewarm water in a tub for 15mins before you take a dump

Buy a massage machine to help relax the back,buttocks

Do not bend over or reach out for things

Sleep on your belly or side

Use preparation h or any other for a few days, tryuse an enema the first day if you cannot go

Follow this and you will get better

I had my spincterotomy 4 weeks ago I am just now feeling improvement and quit bleeding. It was almost more than I could chew.
