I am looking into a procedure for enlarged prostate. MY Urologist suggested the (TURP) procedure but after reviewing on-line I decided this method was too invasive. Any advice?

Stay away from butchers who recommend TURP. Find an experienced HOLEP surgeon:

Definitely want to schedule HOLEP for my giant prostate and was referred to Dr Das in Philly. However I have just noted a Dr Bandi who is in my local Washington DC area. Does anyone have experience with either surgeon and who would you recommend? My feeling is that Dr Das may have more experience and I am OK with going to Philly to get the best experience. Any advice is most welcome! Thanks.

Two questions you should ask of each surgeon: where they received HOLEP training (e.g. from Dr. Lingeman) and how many HOLEP procedures have they done?

A couple of recommendations to you: if going to Rochester from long distance - plan to stay in area hotel after discharge for three days at least. Wait to travel until you are comfortable that things are stable or at least know what the near term 'normal' is for you.
Second- arrange for followup with your urologist on your return. I found it very helpful to have discussed plans before I left. Interestingly my urologist was very interested the procedure and welcomed my feedback.

Thanks for the reply. Yes I definitely will ask those questions! Nervous about getting anything but Holep seems the best option.

I live in San Francisco Bay Area. Early this month I had HoLEP done at UCSF. Dr Marshall Stoller, Dr David Tzou and Dr Yi Li worked together as team for my BPH consultation and treatment. So far I am quite happy with the result. I am pretty much back to normal life right now. As far as I know UCSF is the only credible hospital in San Francisco Bay Area can do HoLEP.

Blay, I am in the process of doing due diligence about how to proceed with addressing my moderately large prostate as well as bladder stones. I've come to the conclusion that HOLEP is the best overall prostate reduction procedure and, accordingly, I have begun personally interviewing HOLEP surgeons.

I went down to Washington in early October to consult with Dr. Bandi. In response to my questions, he said he has done about 200 HOLEP procedures, and that he never formally trained with Dr. Lingeman, but had observed a number of HOLEPs performed by Dr. Lingeman.

I spent close to two hours with Dr. Bandi and his team, including the interm who did the preliminaries.

Dr. Bandi is very straightforward and direct, almost to the point of being blunt. I, personally, am OK with those traits. Some other people may not be.
As I said, I am still in the process of personally interviewing HOLEP surgeons, and have not decided whether or not I will have my HOLEP performed by Dr. Bandi.

I was referred to Dr. Bandi by My local urologist. My prostate measured 240grams and I had prior radiation. I had a Suprapubic tube as I could not urinate. My procedure went great. The urine was clear next day and I was discharged home. He removed 180 grams of tissue. I got my suprapubic tube removed a week later. I could not have a better experience. I had to contact his office a few times to make sure my progress was going well. Everyone was so compassionate and helpful. If you have a local doctor who performs HOLEP, I would go to him. There are issues like infection, incontinence and bleeding. If you go out of town, make sure your local urologist is ok with it and willing to manage any post-operative complications. I did extensive research and it seems like the learning curve is 20-40 cases. You have to feel comfortable with your surgeon. I was very comfortable with Dr. Bandi. He always had a smile on his space and spent a long time explaining the surgery and its risks. He said he learned the procedure to give his patients the best outcome even though it takes him longer time to perform these compared to open or robotic prostatectomy and the reimbursement isn't much higher, I think he practices evidence based medicine and is straightforward in his answers and very much to the point like someone else said. He was a godsend for me and I am so glad to have found him. Good luck with your search.

I had my surgery done in January 2017. I flew from Raleigh, North Carolina to Washington DC to get my procedure done. The whole experience was wonderful. Dr. Bandi Has a smooth and streamlined process for out-of-state patients. You can contact his surgical scheduler Charlene by calling her or emailing her. You can send all of your relevant medical records to her. The doctor will review the record and will let you know if the procedure can be scheduled directly or if he wants you to do any additional testing or be seen by him before the operation. Usually, he wants to see a recent PSA within 1 year, prostate related medications and copies of prostate related procedures like biopsy, ultrasound and cystoscopy. I was a candidate and she emailed me a brochure explaining the procedure and perioperative expectations. He likes to do the Prostate ultrasound himself to measure the volume. I had my surgery scheduled on a Friday. I saw the doctor the same Thursday morning and had my ultrasound done. All of the preoperative blood work and authorizations were done by Charlene through my local primary care physician. The surgery was performed on Friday morning and I was discharged on Saturday. I stayed another night and went back home on Sunday. I agree that keeping your local urologist in the loop is a good idea in case you have any post-operative complications. I had no problems. I had my HoLEP procedure performed by Dr. Mitchell Humphreys at Mayo Scottsdale on Thursday, 10-19-17. Can't say enough positives. The Mayo team, from Dr. Humphreys himself to his Resident (Dr. Zell), to the Anesthesiologist (Dr. Ammon -- highly recommend), to his office staff (Amanda and Alexandra), to the PACU nurses, to the floor nurses [Cis was amazing and made my stay - including the dreaded (at least to me) catheter removal - as relaxed and stress-free as possible] worked together seamlessly like a well-oiled machine to make it as positive of an experience as possible. My pain level was negligible. I was up and walking within an hour of release from PACU. After catheter removal, there was significant blood in urine, which I understand is normal and will dissipate with time and flushing by drinking plenty of water. I also was passing some clots that caused them to keep me a little longer than usual to monitor. When I voided a couple of times with no significant clots, they released me last night.
My only negative so far is the level of incontinence I experienced after voiding while in hospital. It was like a faucet with a worn out washer that, after being turned on high and then shut off, continues to flow at a slow but steady rate. I went through 5 Depends in a couple of hours. After getting home, this symptom seems to be somewhat better. I am hoping that it continues to resolve in favorable direction as time goes by.
It's now noon on 10-21-19 and I have no pain or need for the pain meds; relatively minor stinging on urination (but I have not felt it necessary to take the pyridium that I was discharged with); am voiding approx. every hour and a half (although I am drinking a lot and only got up twice during the night, when I slept for 10 hours because there was no sleep in the hospital.I also was discharged with an anti-spasmodic that I thankfully have not had to use. Running a low-grade temperature of 99.5 -- will monitor but am not overly concerned.
If it is helpful to others, I will continue to update on my progress.

Thank you! I have an appointment with Dr Das and will also schedule one with Dr Bandi since he is local. Even though he has less experience than Dr Das, the glowing reports I see here and his proximity to me may make Dr Bandi the best option once I visit both.
