I've had this for 25 years. The dry skin itches intensely and doesn't flex as much as regular skin. After scratching it off, the skin underneath is raw and red and sometimes bleeds. After years of scratching, the rash has spread in almost a straight line up the right side of the scrotum. The doctor called this a "scratch-itch" rash, saying basically that the scratching caused the skin to itch and the rash to spread, so you scratch it more, etc. The biopsies didn't give a clear answer other than this. Occasionally I'll get a tiny pimple-like thing with a tiny head of hard white skin then I'll know it has spread again.
In several spots where this has been going on the longest, the skin is raised and thickened. The same areas tend to itch most every 2 or 3 days, especially as the dry skin grows back after being scratched off. This leads me to think it's related to psoriasis since the skin grows back so fast. The same spot of skin can itch, be scratched off and feel better, regrow the dry skin, and be scratched off again and again. The skin that peels (is scratched) off is absolutely dry and brittle - not like skin at all or even like skin that peels off after a sunburn. The sites where the skin is raised and/or the scratching has been the most intense/long lasting sometimes hurt for a few seconds, which is a bit worrisome.
Needless to say, this is an embarrassing and time consuming problem. Part of it I think is caused by the warm moistness of the
scrotum tucked tightly in the underwear. I usually park the penis on the right side of the underwear which I think crowds the scrotum and causes more chafing and perspiration than on the left. I think this moistens the skin and makes it more susceptible to this condition.
I've tried everything, including going cold-turkey (no lotions) for awhile after becoming frustrated with lack of success. I would not recommend this. Despite my hopes that I could beat this with will power, going without using anything has made the situation a bit worse.
Here's are some things that have worked in the past that I plan to go back to:
Zinc Oxide - the white cream that swimmers sometimes put on their noses - spreading this on and around the affected area in the morning and evening helped. I've read that skin with this condition is low in zinc. The cream also has a lubricating affect that reduces friction. This absolutely works as it cleared up a spot for me a few years ago. However, it only works if you can stop scratching. The spot it cleared for me wasn't one of the hot spots where the itching was unbearable. If you keep scratching nothing will help you.
Gold bond powder - in the morning, using this in the crotch area dries it out nicely and it has a tingling sensation that is refreshing. I believe keeping the area dry with the powder reduces the swelling and moistness that makes you want to scratch. Also, perspiration has salt in it and salt on raw skin irritates it and makes you want to scratch/rub. Again nothing will work if you scratch so anything that can be done to stop scratching is good.
Looser underwear - I have not tried boxers but underwear that fits properly and allows plenty of room is the best.
Cortizone 1% Cream - I prefer the cream to the ointment as the ointment somehow causes the skin to itch a bit. The cream is absorbed into the skin better.
Generally I applied the creams/powders once in the morning & night but a midday application couldn't hurt if you can do it at work.
I plan to try using a daily vitamin with zinc and some other nutrients as recommended on another post. I hope that some combination of all these things helps.

Guys this is a very concerning issue, touched a babe recently but i dont see how that connects....
another thing is i Shaved the region....
another thins is; in other to protect myself from the lady i laid with from some funny skin like patterns on her body i used gallic, onions and a leaf; squeezed the juice and used it on the spot and then came the big thing, i have been trouble...
but i just found out something try keeping the place dry.... like sleeping naked with the tea bag hanging hanging up
may be this would work

This just started happening to me too! I really want it to stop!
Wow I thought I was a rare case and certainly feel much better that others share my issue. Its kind of embarrasing to disclose this type of condition so I've searched for my own remedy and only found a temporary solution. Benedryl anti-itch gel mixed with some lotion stops the itch, and dryness long enough for me to get through the day comfortably..keep in mind this is only a temporary fix not a cure..

im having the same problem as a lot of you on here. its rather new to me it started with having to itch contantly, looking up the symptoms its similar to psoriasis but occurs on the testicles and not where said. it hurts but feels good to scratch it. im a cashier for 8hrs a day and i shower on a regular basis but it gets kinda gross down there, there ends up with an unpleasant smell from down below. i have yet to use any creams or treatments nor have i seen a doctor about this yet.
Hey, i also have this problem, not as bad as some of you. But, nevertheless, i think i found a way to cure this. (at least for me). alright so you have your dry, itchy scrotum :(sadface. here is an easy way that should fix it, you might have to do it for a few days. So just get some moisturizing cream and massage it in to the affected area. it'll burn for like, 2-3mins but afterward it won't even bother you. if it itches DO NOT ITCH IT WITH YOUR NAILS, just kind of rub it or pinch it. also i think this condition is due to soap or other things of that nature being rubbed on your scrotum, so try and stay away from them. Also, do not masturbate or practice self pleasure at all during the period of time in which you have this, it can make it alot worse. I really hope this helps..
I get this and I found out years ago after visiting doctors that your body is maintaining high levels of yeast type bacteria. I get same problems. I also fight dandruff and such. Do this.
Run a hot bath. Pour a 1/4 or so of Apple Cider Vinegar in the water and soak. Do this daily
Next, go out and buy Oil of Oregano pills AND drops. Take these regularly to keep levels down. 3 - 4 pills a day and couple drops under the tongue.
Also, get your doctor to prescribe 2% Nixoral shampoo. Wash the area down there with it as well as any other part of your body with it.

I have the same thing or something very similar. And my crotch itched super bad. I shaved my crotch with electric hair cutters with a #1 guide on it, then put Aloe Vera Gelly (Lily of the Desert brand) on my crotch where the itch is 1 or 2 times a day. The itch is mostly gone now. It still itches sometimes but my life is livable now. The itch used to be so bad I was thinking of killing myself. I hope this information helps someone.
I am suffering the same symptoms as other posters but have one other that hasn't been mentioned, just wondering if others have this too. When the skin on my scrotum is itchy, but before it flakes it feels like the itch is below the surface of the skin if that makes sense. Everything else is like other people on here with the skin appearing very tight and thin over the itchy patch. It only really seems to affect me during the summer months as far as I can tell. I've been putting up with this for about 4 years now and nothing really seems to help long term. The only thing I haven't tried is the Aloe Vera Gel so may be will give that a try.

Agree with poster who talked about Zinc Oxide. That seems to be the best thing for restoring the skin and the the area around where you've been itching. It doesn't quite stop the itching by itself though. That is a problem because as the skin heals it is still soft and one good scratch puts you back to square one. What's the best way anyone has found to stop the itching?
