I'm more liable to think it is the enlarged vein... and by that I mean a varicocele. I don't think it's a fungal infection, but your guess is as good as mine. Have you also asked about autoimmune attacks in that area?
If the pain has reduced, it may be a coincidence and your body is just slowly healing. It happens to me, as well. I go through the occasional row of dull aches and pains one day and then the next I don't feel anything at all, as if it never even happened. My sex life has suffered immensely due to this since I sleep with a heating pad on my right pelvis or sometimes on my testicles, which has most likely caused damage in terms of low testosterone production and infertility.
I have had this for almost two years and had pain so bad that they removed the left testicle but pain in the right side is still there.
They found cyst in the right side and I had surgery to remove them but still have discomfort. I have used pain re leaving gels and rub it into the testicle and that seems to be the best so far but it never goes completely away. Hope you get relief soon as this is no fun and has basically ruined my life.
Hi JCA. I had a nerve block on the left side several months before they decided surgery was needed. The nerve block only lasted a couple of days and basically it is just an injection of the same stuff they use when you have dental work. I have looked into a lot of things and presently trying hypnosis. I have just started and will let you know if this works. The idea is that the mind can block most of the pain signals. I saw a woman have a tooth drilled while under hypnosis and she felt nothing. Interesting concept and will let you know how things progress. I was told that I should have reasonable in 4 secession s and if not then it will probably be better to stop.
This is such a depressing ailment and the Dr's seem to treat it like it is nothing. Not that I would wish it on anyone but it would be justice if it happened to one of those guys that think it nothing. I probably sound bitter and I am. I almost feel the removal of the right side is the best option but I will probably hack it out for another year before doing that

I have had the pain for a couple of years and the left side was so bad I had it removed and all he said was it was a mess. I thought he was just going to do the epididymis but he took everything. The pain continued on the right side which was not as bad as the left and another Urologist said the bladder restriction had cause back up urine infection and he did a turp proceedure and lots of antibiotics, again no relief, but I do pee a lot better. I went to a third Urologist and with an ultrasound they found cysts in the right side scrotum and removed the cyst. The surgery was 4 months ago and still discomfort but not as bad.
He told me that it will take 9 months for that part of the body to stabilize and just use anti-inflamitories and live with it.
I feel for you as I know there seems to be no end to this except the big one. I tried many of the juices and anything else that I could find but to no avail. I was told that this is the most sensitive place on a male and if it was somewhere else it would not be an issue but it is not somewhere else and the medical society just seems to throw guesses at it.

See, this is where it's upsetting to read: That the surgeon decided to remove the testicle outright. I mean, was this a risk that was explained before the surgery? And what was the reasoning behind it? Sometimes, I think the specialists are just trying to cover up their mistakes. An epididymectomy is not an easy procedure and while they're excising the epididymis, they can cut through a blood vessel by mistake, which will then require the removal of the testicle since its blood supply has been compromised. The first couple of specialists I saw were ready to do this surgery on me right away without asking any more questions...
I'm just wondering what triggered this for you (e.g., an accident, an infection, something) and where the pain was originating from (e.g., the testis, the spermatic cord, the epididymis). Did you have any lumps, bumps, or intrascrotal swelling? Did the testicles hang differently (lower, higher, horizontal)? Were you recommended to wear scrotal support? Is it a sharp, acute pain or dull and constant, like a throbbing toothache?
As far as hypnosis goes, this was something a physical therapist also explained to me: The more emphasis and thought you place on the pain, the more that part of the brain becomes larger and larger, and becomes more fine-tuned to sense pain. In other words, you have to learn to ignore it or at least not be afraid of it, and find ways to cope with the pain episode.
For myself, I think I've managed to heal over the year, but there is something to that whole "don't be afraid." At first, just touching my scrotum would send shooting pain up the inguinal area. Later on, I managed to feel around whenever I felt aches and pains. Putting a heating pad near the area also helped distract my mind from the discomfort by focusing on the warmth in another area. Granted, I also placed a heating pad at or near my scrotum, which is not something I recommend (although it worked in calming down the aches and pains).

So I had 10 days of cipro it was ineffective.
I decided to research and self medicate myself this is what I ended up taking
10 days of 100mg doxycycline
10 days of zinnat 250mg
entatnum 25 mg (pain relief)
10 days cipro 500 mg first 10 days ineffective
This was the best comibined with doxycyline and zinnat 250mg intermuscular shot cefriaxone.

after reading various post on this forum, i concluded to write my own experience with epididymitis. any questions, queries, requests, comments, suggestions, corrections are most welcome. this all started sometime in sept 2013 when i felt that my right testicle is bit heavier and i was consistently reminded about it. then i thought of getting it checked up with a clinical doctor. he said everything looks fine . this thing continued. with pain slightly moving towards upper side especially when i was experiencing arousal. then pain shifted more towards inner thighs over months. during the period i visited various doctors who said everything is fine. since doctors werent serious about it and pain was very mild and tolerable though disturbing i did not thought about it much. the real problems started in june 2014. when i felt real pain in inner thighs , lower abdomen, sometimes back side also and the back of my scrotum . i got blood tests, urine tests, sonographies etc. everything was normal but the surgeon told me that i am having orchitis epididymitis. he said he cant do anything for it and cant operate since i am young (21) and if he operate i may not be able to father to any child later. i took the medicines they gave. no relief till nov 2015. in between i took homeopathy as well but in vain. in nov 2014 i visited another doctor. who gave me a normal report but gave a pain killer for 10 days. and then i also reduced the masturbation to very minimum which helped me a lot.i have been masturbating at a rate of 1.5 times a day since i have been 12 years till june 2014. i dont think there is any problem in that . but the real problem i think that caused me great amount of pain was how i masturbated. i used to keep the penis in its normal position that is bent downwards and will stimulate the penis head only with my thumb (with the part just below the nail ) and the four fingers in its semi erect condition. in that way i could not gush out the semen properly and i think the internal machinery suffered over time due to that. that is the reason i think it all happened with me. i am virigin with no sexual experience ever. so stopping masturbation helped me a lot. i recovered from each and every pain to a great extent. i am not taking any medicines. i am currently able to control my masturbation to 1 time in 4 days. it is very dificult for me to do that because i have been very active earlier. but its worth it when u want the pain to go away. so this was the only non- medicinal way i could discover. further i wear scrotal support which helps too. i even deduced why i do not get pains when i am not masturbating. when ur testes are making sperms they hang lower from the body to decrease the temperature as the temperture near the body is relatively higher for such process. but when ur testes is already full of it they are not making the sperms at that pace and is relatively closer to body since they can manage to . now when u dont masturbate u have enough store of sperms making ur scrotum stick more towards ur body, that way the nerves and pipes do not get stretched away and ultimately relieve you of the pain .
At first it was realy painful, but what really is helping me ., first, cipro 500 then , i take a lot of yoghurt to fight bacterias spreading in my body, take plenty of water, drink dandelion tea atleast three times a day, dandelion tea has no side effect , i had a great improvement atleast my epididymitis lessen for a bit and doctors say treating epi takes a longer time so be patient. Do hot sitz bath to change the circulTion of blood down there so antibiotics could work well. By also adding hydrogen peroxide on hot sitz bath would eliminate bacterias .. For me, Avoid masterbaiting because wet dreams would come anyway. Im not suffering from pain anymore and im just expecting for the lump to go away..i would try to update you if things went well. Just dont stress out your self.. Do not check your testicle every day coz u will not see improvements., check once a week or once in 2 weeks.. Avoid junk foods ,coke, alcohol, smoking, better wear underwear to lift testicles rather than cycling shorts.., pee if you felt peeing, ateast u pushed out some of the bacterias. Just bhink positive and never stress out ur self on thinking if your gonna heal or not.. Just remeber that it could last for months. Recently though I experienced sharp pain underneath my right testicle that was far worse than any pain there I've felt before. I went to the clinic and they said this time I had an infection of the epidymitis and prescribed antibiotics. It was most likely a back up of bacteria from sex or urine that did not completely empty out as it should have.
I worked out to hard the first few days and things got worse. I finally decided to lay low and do nothing but RICE (rest ice compression elevation) this weekend. That in conjunction with icing it down constantly and taking two warm baths daily with Epsom salts (highly recommended) and its calmed down considerably. I think you also have to keep your anxiety level down as well. Getting upset seems to aggravate the condition more for some reason. Also I can't emphasize enough that you have to keep the "pipes" clean. Always urinate after sex or masturbating and if you don't have sex on a regular basis you have to take matters into your own hands by dumping the sperm out so it does not back up and become infected causing epidymitis to flare up again.
