Hey man. You aren't alone on this. I've had this exact same issue for two years now. I notice that after I have ejaculated, give or take 30m-1hr later my testicles seem smaller and the epididymis on my left side seems tender to the touch and almost hard like. Now realize too that after ejaculation, all that down there does expand and swell a little. That's natural, but mine can stay in this state for sometimes a day or two. Also to add, a varicocele can be towards the testicle as well. There are veins that run down to where the vas deferen connects to the testicle. From what I've been told from ultrasounds and a very experienced urologist is that I have a small varicocele and if this problem persists in discomfort, then to take Tylenol and ibuprofen together. I can at times go weeks without the issue, but sometimes it floors up for weeks as well. All in all, it's nothing serious, just another issue we have to deal with.

I have exactly same problem over 2 years now.
I think in my case it started with laying on one side for a long portion of day and sitting on wooden chair for hours.
My hip hurts and I have tight hamstrings but I don't have problem in walking for mile. After ejaculation my left testicle shrink and hurts and a hard coil can be feel around it.I did all tests and all come negative.
I took advice from a local indian non medicine doctor (guru).
He told me to eat raw garlic and honey in morning before 1 hour of breakfast along with couple of yoga. Also he told me pull both testicles down while standing by using thumb and index finger by making Ok sign. I am using it from 2-3 months and it really helped me. Also before doing ejectluation I tie both testicles with a doctor tape in same position as Ok. so after sex all vain will be in attached position.
