A friends husband sprained his ball sac. I have never heard of it and am wondering is it even possible

I've never heard of it happening but there are muscles in the scrotum. The cremaster muscle raises/lowers the testicles and the dartos fascia muscles lie just under the skin.
What specifically did he sprain?
