for up to this: went and saw my doctor tonight (again!) as it flared up not long after the above post. he took a good look, listened carefully and weighed up all the options. He eliminated predundal nerve damage, prostatitus, STD's and more importantly 'it being in my head'. He concluded that what i experienced was a flare up and that theres a nerve inside the glans head that for whatever reason decided to fire off. Regular pain killers brought it down again so he advised continuing with those for how ever long i need to as they seem to be effective. He did stress that theres no way to predict how long this will take to sort itself out, could be days, weeks, months, etc
So im going to take the paracetamol, stick two fingers up at it and get on with my life.

ok follow on to previous post. For the most part it feels better than it did a few weeks ago. yesterday it carried on 'grumbling' off and on so I decided to 'bash one out' and see what happens. fortunately everything worked ok although the urethral did feel like it clenched a bit hard towards the end. this morning it feels a tender so my best guess is that here is some inflammation in there from damage but not from an infection. hey-ho :)
I think ive found the cause, It's 'male genital dysaesthesia'.

I've been dealing with a similar issue for a couple years now. For me, I believe it's chronic pelvic pain. I was prescribed physical therapy for pelvis pain and its helped quite a bit.
Basic idea is that men can store tension / stress in their pelvis (similar to a tension headache) that causes an over activation of muscles / nerves in the pelvic area.
