I am a long distance and racer cyclist and I have been suffering from a rectal bleeding and burning since June 5th and used Prep-H with no help. Finally I saw a Proctologist/Surgeon on August 24th and he suggested Colonoscopy and also said that I have a posterior midline fissure and prescribed Nifedipine 0.3% & Lidocaine compounded cream.
October 7th procedure was done, colonoscopy showed no polyps, sigmoid spasm ( Dr. was not worried about the spasm ), 3 large internal hemorrhoid banding and a Botox injection for the fissure.
I had a follow up 11 days after the procedure, and the Dr. said that most likely the hemorrhoids already fallen off and the fissure is healing ok, but I am still in pain and blood on tissue and some on the stool ( bright blood ) after 2 weeks post procedure, my bowel movement is very painful and after about 1 hours from BM severe burning and pain starts for about 3 to 4 hours. I am eating fruits, veggies and drink a lot of water, and off the saddle for 3 months per doctor.
I am suffering now from incontinence, per my Primary care doctor the incontinence is from the Botox shot and that should wear off within 3 months, I had pelvic MRI done 2 days ago with very clean results ( no abscess ), however, the severe pain that is paralyzing me for 4 to 5 hours after BM is of a great concern and is affecting my life and work tremendously, and the specialist does not know where the pain and the pressure is coming from. Has anyone had this happened to them? I am seeing another proctologist for a second opinion, please help me.

I have had this excrutiating pain for nearly 4 months, i am almost certain that this started after getting an anal fissure from having constapation. But since this happened the problem has spiraled to going for a poo and having escalating pain after the motion for hours after, sometimes there is no pain during the motion and i sigh in releif, then bang, my anal passage contracts tightly and i have pain that no pain killer helps with (even oral morphine). I have checked my passage before motion, it is accomodating to my finger, when the pain kicks in i cannot examine myself due to severe tightness. there is no external damage or soreness. my only sources of releif are 'pretending to poo on the toilet' walking or for the best releif riding my bicycle, (the releif from walking or cycling takes about 5 mins and was realised by pure chance) i cannot sit anywhere other than the loo and lying down offers no releif. the pain is not taken away by cycling any quicker, however it becomes bearable during the ride to a level of discomfort, once i dismount 5-10 mins after the pain will return unless the usual 'cool off period' has elapsed. occasionally using a lubricated finger i can get temporary releif by tricking the anus in to thinking there is a motion. i belive that there is a muscle spasm and i have booked to go back to the doctors tomorrow. this has ruined my life as i cannot possibly drive my 100 miles commute to work. i have an underlying gaul bladder issue that led to hospitalisation due to pancreatitis caused by the gaul bladder. i will point out that high fibre diets have been mentioned, i have never before had diet issues or problems passing poo, it does not matter if my bowel movement is very hard or diahorea the after pain is the same. what does matter is that if the poos are hard the pain during movement is more severe and pain during poos that are runny is very minimal. when poos are difficult to egress i have found that using lube before pushing helps, if the first push is unsuccessfull, stop, and using a enema bulb (dooshe bag) squirt warm water into the anus that will lubricate the motion. please forgive my openess but i have reached the point of dispair that i just dont care i hope someone can find a soloution.
After experiencing the same pain, but, more intense over time, it was found I had an ulcer on the illum. Now, many doctors over 40 years, I believe I finally found the treating doctor and the treatment. Taking Rimicin, Dobson, Entecort, so the pain and symptoms now ocur probably once a week. Now, to discover why is the next goal;-)
Its now 6 years since you posted this. Have you managed to find the problem and solution yet as I am suffering from the same problem and doctors are so far clueless. Any help would be much appreciated.

I had some piles removed a couple of years ago and ever since had a delay of about one hour after BM then pain from anything from 3 to 8 hours. i have had 2 camera's up my back side and one down my penis but they fiound nothing, Now i have extra symptoms of pressure on my bladder after BM and wind.

I think I can help. My advice is to step out of the cycle of worry and self-serving doctors, for at least a week and see what happens.
I, too, had been recently suffering greatly from delayed rectal pain after bowel movements - lasting sometimes the entire day so that even my tailbone and upper thighs ached and felt weak. My life was turned upside down by this for over a six week period. Worry? You bet. Doctors...not yet.
Instead I researched the best source on this there is: online blogs by other sufferers from this same condition - such as yourself. I then began the following regimen that cleared up this condition - completely - within three days.
Went to the supermarket and purchased the following items:1. California Pitted Prunes 2. Organic Psyllium Fiber (Konsyl Brand) 3. Generic Stool Softener 4. Organic Flaxseed Meal (Bob's Red Mill Brand) 5. Bottle of Witch Hazel 6. Hemorrhoid Cream
At the outset, snacked on the prunes, eliminated beef, sugars, cheese, bread (not entirely), etc. from diet, loaded up on vegetables, fruits, probiotic infused yogurt and drank and continue to drink spring water throughout the day - I cut down on caloric intake in general.
Twice a day, large glass of water with the Pysllium Fiber. One Stool softener tablet (100mg) per day.
After first bowel movement and shower soaked my rectum thoroughly (flat cotton swabs) with warm (not hot) water and coated rectum lining and surface insides with the witch hazel (advised by a anal fissure specialist online); and swabbed rectum surface with hemmorhoid ointment before dressing. Repeated this process before bedtime.
Ate a few bowls, daily, of organic raison bran and oat cereal (low calorie) sprinkled with two tablespoons of the Organic Flaxseed Meal. Also ate probiotic yogurt, raw almonds for snacks, salads and fish for meals, water, water, water.
Cut back on caffeine and alcohol (one cup java, afternoon tea and one glass of wine per day).
Stuck to this regimen and procedure and will continue to do so - the prunes are a must throughout the day....and water.
The first morning after beginning this regimen I awoke to my clockwork, post-coffee bowel movement, this time before my coffee, and defecated large and soft, but solid. Next day an unusual second bowel movement, as large, soft and solid as previous day, followed an hour after an initial soft bowel movement. Pain free the entire two days for the first time in six weeks. Third day, one bowel movement only but again, large, soft, solid. Pain free, feeling fully restored, energy revitalized and general feeling of intestinal emptiness not felt for months.
My conclusion is that my (and perhaps your) rectal pain was caused from blocked feces within the colon - built up over time due to poor diet, stress, etc. - and that triggers long lasting spasms when we are in the cycle of incomplete bowel movements - and that we may even believe are complete. This, in my view, exasperates anal fissures, hemmorhoids, etc. and over the long term can develop into a serious problem.
I am 56 years of age, active and fit (running for 30 years) and, of course, dealing each day with common stress, professional and family problems as we all do....But, we can maintain and restore our health with self-awareness and common sense approach.
