Mine and sillywilliams sound the most alike. My pain is immediately after ejaculation, also near the head of my penis. I also have a less painful sensation more in the urethra in the shaft of my penis. I always have to urinate almost immediately after ejaculation. Again I have the same pain. I have experienced pain so intense that is literally brought me to my knees. The pain is so intense I grab my penis and sort of squeeze it. .I don't know why, it doesn't do anything. I guess it's like having a stomach ache and holding your stomach. I too have the sensation or feeling need to urinate again and again. I go the bathroom and all I can do is dribble, this goes on for a while. My discomfort has probably at the most lasted about 15mins, after the initial pain. Yesterday I didn't have sex or masturbate, but I still had the same discomfort. It was nothing like the pain I previously described.
I've had this for 4 years. It started after I had been evicted (massive stress) and my first experience of it was painful ejaculation, which normally means prostate problems, so I got my prostate checked and it's fine - no cancer & not enlarged.
My doc sent me to a urologist who recorded poor urine flow and bladder retention and he prescribed Flomax (tamsulosin hydrochloride) to relax my pelvic floor muscles and 6 weeks of antibiotics. He was interested in how stressed I was..
I still had symptoms but less frequently, I'm guessing because I wasn't as stressed - because all I needed was antibiotics according to them. A few months later I was getting the same level of pain.
I've had all the same symptoms as all the other sufferers here. Debilitating discomfort after peeing after ejaculation. Avoiding a full bladder before sex, being as gentle as possible, and waiting 40 mins after sex, all seem to help. I don't drink and I didn't have this for the first 40 years of my life. This is something new, to my body anyway. The only 'new' thing to me is ongoing seemingly neverending stress.
Having read all the comments I'm pretty much convinced most if not all of us have Interstitial Cystitis (the wiki entry is good). If I want to find out if I have IC I have to have a cytoscopy to rule out other problems I know I don't have, and I just can't face it.
Doctors won't tell you you have it until they've ruled out all the common bladder/prostate/urinary tract problems - all of them. Which means they'll do cytoscopy down your urethra and biopsies from your prostate til you pee and cum blood - causing anything from urinary incontinence and urethral damage to complete loss of ejaculation as one commenter said happened to him.
Only once they've eliminated the established normal problems that they know about will they consider diagnosing you with IC - because they say there is no test for it. But by then you may regret going thru all that pain, suffering and stress. Also, there IS a test for it - injecting potassium chloride into your bladder through your urethra. If you immediately get those familiar symptoms that us sufferers here know only too well, then they'll diagnose you with IC...
I'm avoiding all that and trying to find a urologist who will accept my own diagnosis and give me what I need - pills to relieve stress. In the mean time I'm doing Kegel exercises to strengthen my pelvic floor, and avoiding stress and self-doubt wherever possible.

I have this same problem. Its ok you dont have anything wrong. A word of advice, dont chug a water bottle before or alittle after sex or masterbation. After ejaculation, the urethra is simply irritated, give it time for it to equilibriate( 30 mins to an hour) before you urinate. Unless you are really dehydrated, try not to consume large amounts of fluids if you are plannning to have sex or masterbation. Cheers!!!

Since nobody on this page seems to know any causes, I think I can help as a guess. Basically, urine and sperm do not mix. The urine does not neccesarily kill the sperm, but it does something to it. Whatever it is, I think it is to do with the urine mixing with the semen. So if you want to prevent it, then just do what a lot of people have suggested and don't ejaculate if you need to urinate. Simple. Hope this helped.
I'm 28, have had this since the age of about 12. I first started noticing this with masturbation at a young age but these days it more commonly occurs with alcohol. Sometimes sex involved causes it but I can just as easily get it just from alcohol (with no sex/masturbation). I just wanted to highlight that as most people only seem to get it from ejaculation. I definitely have the ejaculation experience as well, but quite often I can get it just from a night on the booze. It is rare though, maybe a couple times in a year. Perhaps 1 out of 20 drinking binges, hard to say exactly but it happens rarely. When it does happen though it is terrible. Once it passes that's usually the end of it for good.
People speak about dehydration as a possibility, but I drink so much water daily, at least 3L. I realise when drinking alcohol I'm going to be losing more water than normal but I just cant be sure it is dehydration. I also drink a few coffees each day + preworkout supps so I'm not light on caffeine and caffeine has never given me the issue.
One of the reasons I just read through all 18 pages was in the hope that someone would have also noticed this...
I've done cocaine twice in the past, and both times it happened. The most recent time was the worst experience I've had with this ever. I had the symptoms about 4 times during the night whilst on coke (no sex either)! The last one of the night was the worst too. I was drinking alcohol but not a great deal, and the last few hours of the night I was drinking a lot of water (I was being conscious of the possiblity it was a dehydration issue). Despite all this water I still had it??
It's also worth mentioning that I do believe I have a weak bladder. I urinate so often, but at the same time I drink a lot of water + coffee. Despite this I do think I still urinate more often than "normal".
Some people mention Interstitial Cystitis and Prostatitis. The people who have been treated as "Prostatitis" with a long course of antibiotics (1-2 months) - did this resolve the issues for you long term? I'm guessing you won't return to this forum if you don't have it anymore though :(
Would love to know if someone triggered this with anything other than alcohol (i.e. cocaine).
Oh and my theory most recently was that there is a change in PH levels when drinking alcohol or other substances. I thought maybe this PH level could create extra acidity which causes the burning for a small time? Has anyone ever done a PH test whilst having the symptoms?
I get the EXACT same feeling as the question maker. But I NEVER drink alcohol, so it has to do with something else.
