I had this problem for several years. I tried everything. I saw more than one dermatologist. I used numerous shampoos and a few antibiotics. Nothing worked for long. It always came back.
Try drying the back of your head with a hair blow dryer after showering. Dry it thoroughly and consistently for a week or two. It improved my condition substantially.

Hey guys, im glad to know that I am not the only person suffering with this terrible condition. Mine started out from the time I was 20, I am now 27 and after numerous visits to the dermo, anti biotics and creams, I still have acne at the back of my head. My dermo would tell me that it could be that I sweat from the back or that I cut my hair too short...but the acne would still come up while my hair was long...so I dont understand. The antibiotics I have used are Purbac and tetralysal... In most occasions, I was told to use them for a period of 3 months or so. It would work and I'd feel so much better but give it a few weeks after the treatment was done and they would re-appear. In the last few months, I have been taking an antibiotic on my own account (purbac)...just the other day the bottle was empty and what do you know... the acne came back. At this stage I am so frustrated because it really affects my everyday life... besides the pain, I am embarrased that people would see the red spots and question me. I somehow feel that this problem could be internal but have no clue on what is the next step to take.
make sure that your pillows and sheets are clean and use mild soap. sometimes it gets unbearably itchy specially since it is always in contact with your hair. don't scratch it, and make sure that you always use mild shampoo

I've had this problem for about 2 years now. I'm 43 and have plenty of stress in my life lately. I've been to the dermatologist and he prescribed doxycycline hyclate, which I quit taking due to the obvious fact that it's terrible in the long run. He suggested I might have to take it in maintenance doses for a very long time, which I was surprised to hear from a 'doctor.' Anyway, like all of you, I google'd a lot. Found suggestions regarding gluten. I decided to give it a try and sure enough, cutting gluten did help quite a bit, but I still had breakouts, albeit less often. I also cut beer out and that also helped. I still felt like it was occurring too often for such drastic changes in diet, etc. I then tried vitamins... specifically B-complex, D and a multi-vitamin from Costco for "Hair, Skin and Nails." I take one of each in the morning and then again in the evening. The difference has been huge. Before, I used to get these inflamations which would last for days and would be incredibly painful. Now I just get the occasional minor pimple which lasts a day or two. I have noticed that these still occasional pimples cooincide with times where I eat something not so healthy (ex: french fries... the oil is contaminated with whatever else has been deep fried in it).
So in summary... 1) do your best to cut out crappy foods, especially gluten. 2) stay away from restaurant deep fried foods. 3) use B-complex, D and E (look for the Costco 'skin/hair/nails' vitamin) twice a day. Your body only absorbs a percentage of the B-complex, so even if it says once a day, do it twice.
I hope this helps somebody out there.
